Monthly membership includes:

• Primary care by appointment

• Telephone and video access when appropriate

• Annual visits

• Well woman visits, including pap smear

• Sports physicals

• Sick visits

• Prevention care and planning

• Chronic care management (such as diabetes, hypertension, allergies, etc.)

• Mental health care (depression, anxiety)

• Specialty care coordination

• Urine pregnancy test
